Our Client is seeking a Service Coordinator to join their team!

Job Duties Include:
-Verify eligibility of referred homeless person/family.
-Conduct intake and assessments.
-Develop and monitor service plan for each individual.
-Work w/Housing Specialist to ensure placement in safe housing.
-Refer services with other agencies for life skills training, financial assistance, mental health services, substance abuse services, etc.
-Utilize "housing first" philosophy to ensure services such as mental health assistance, substance abuse assistance, etc and resolve barriers of staying in housing.
-Schedule and conduct regular meetings with clients.
-Record service coordination efforts constantly in client records.
-Data entry, meetings, additional clerical tasks as assigned.
-Bachelor's Degree in Social Work, Human Services, Counseling or other behavioral science, or equivalent experience with education would be considered.
-Knowledge of mental health, substance abuse issues & services.
-High energy and patience in working with difficult clients.
-Ability to work independently and with a team.
-Computer skills using Windows.
-Culturally competent counseling skills.
-Flexibility with hours, some evenings and weekends may be required occasionally.
-Valid CA Driver's License.
-Ability to provide own transportation with acceptable driving record and insurance.
-Bilingual in Spanish preferred, but not required.
